Change to ‘Where needed, we can provide a written report including a risk assessment and personalised falls prevention plan that directly supports CQC Regulation 12 – Safe Care and Treatment. Working closely with care home managers and staff, this can be incorporated into the resident’s care plan, including recommendations for equipment to support safe transfers and mobility – ensuring the whole care team is informed and working in collaboration for the benefit of the resident.
Our physiotherapists assess residents within their own environment – their room, corridors and communal areas – to develop personalised rehabilitation programmes that reflect real daily life rather than a clinical setting. Following each visit, our physiotherapists liaise with the care team on how to safely support individual residents between sessions. Where needed, we can provide written guidance, contribute to or review the resident’s care plan, and deliver hands-on training for care staff on the safe use of transfer equipment, mobility aids, and any other equipment within our scope of practice.
