    Falls prevention & risk assessment.

    A structured falls prevention programme that addresses balance, strength, coordination, walking aid suitability, footwear, and home hazards. Falls assessments are fully documented — producing a written risk report and personalised prevention plan that directly supports CQC Regulation 12 (safe care and treatment) and is available for inclusion in your inspection file.

    Mobility & walking rehabilitation.

    Personalised programmes to improve or maintain functional walking ability, transfers between furniture, and independence with daily activities. Your physiotherapist assesses the resident in their actual environment — their room, corridors, communal areas — and builds a programme that reflects their real daily life, not a clinic setting.

    Specialist physiotherapy for residents with stroke, Parkinson’s disease, multiple sclerosis, and other neurological conditions. Our dementia-trained physiotherapists are experienced in adapting treatment for residents with cognitive difficulties — working at the resident’s pace, with consistency and genuine patience, and always involving family members and carers throughout.

    Targeted treatment for arthritis, joint pain, back and neck pain, and other musculoskeletal conditions that affect residents’ quality of life and functional independence. We use manual therapy, therapeutic exercise, and pain education — all adapted for frailty, complexity, and individual capability.

    Staff guidance & moving and handling advice.

    After each visit, your physiotherapist provides written guidance for your care team on how to support individual residents safely between visits — safe transfer techniques, walking aid use, and how to encourage prescribed exercises. This supports CQC Regulation 18 (staffing competency) and helps prevent injuries to both residents and staff.

For urgent individual referrals — post-fall, post-discharge, or acute need — we aim to visit within 24 to 48 hours.     After every session, your physiotherapist completes structured clinical notes covering the assessment, treatment delivered, resident response, and next steps. For residents on rehabilitation or falls prevention programmes, we maintain a written plan with measurable goals and a clear progress audit trail. All documentation is available to share with GPs, families, and your CQC inspection file. Scheduled contract partners receive regular summary reports on all active residents.
     
    Yes — our team is specifically dementia-trained. We adapt our communication style, pace, and approach for residents with cognitive difficulties, always working with patience and consistency. We involve care staff and family members throughout — providing guidance on how to support exercises and movement between visits. We're experienced working in dementia care homes and supported living settings.
     
    Absolutely — many of our referrals come from family members rather than care home management. Simply call us on 020 8087 3132. We'll confirm with the care home that they're happy for us to visit (we handle that conversation), and we'll take care of everything else. We'll update you after each visit so you always know how your loved one is progressing.
     
    Our service directly supports three key CQC regulations. Regulation 9 (person-centred care) — every resident under our care has an individual, written rehabilitation plan. Regulation 12 (safe care and treatment) — falls risk assessments and prevention programmes are documented and reviewed. Regulation 18 (staffing) — we provide written moving and handling guidance for your care team. All documentation is kept current and is available for CQC inspection review.
