Our elderly care physiotherapy service is designed for older adults with reduced mobility, strength, balance, or independence due to the following conditions. We visit older individuals at home, in care homes, and in nursing homes across a wide coverage areas:

Falls & balance problems

Recurrent or recent falls, fear of falling, unsteady gait, or loss of confidence on stairs and outdoor mobility.

Fractures & post-surgical recovery

Post trauma fractures like hip, knee, shoulder and vertebrae’s as well as joint replacements whether elective or following fractures. We specialise in rehabilitation after surgery or injury to restore function and mobility.

Reduced mobility & deconditioning

Reduced muscle strength, stamina and global deconditioning affecting mobility following illness, hospital admission, or prolonged bed rest.

Stroke & neurological conditions

Rehabilitation following stroke, Parkinson’s disease, multiple sclerosis, and other long -term neurological conditions affecting movement and function.

What does elderly physiotherapy at home involve?

Every programme begins with a comprehensive 60-minute assessment.
From there, we design and deliver personalised hands-on treatment – not just a list of exercises – monitoring and modifying the plan as per progress. Here is what our physiotherapists actually do during sessions.

Home & mobility assessment

Detailed assessment of muscle strength, dynamic balance, joint range of motion, coordination, and mobility. We also assess the home environment – stairs, furniture, lighting, rails and recommend adaptations accordingly, if needed

Neurological rehabilitation

Specialist rehabilitation for recent and old stroke and elderly individuals with Parkinson’s, Multiple sclerosis, or other neurological conditions. The goal is improving movement, reducing stiffness and risk of contractures, and maintaining independence.

Balance & gait retraining

Specific exercises to improve static and dynamic balance aiming at improving stability, gait re-education, and confidence building on different surfaces -reducing fall risk and improving the safety and quality of everyday movement and activities.

Assessment of mobility aids

A thorough mobility aid assessment is an important part of elderly physiotherapy as the wrong aid, or incorrectly used aid can increase the risk of falls. Our physiotherapists check, whether the equipment is right for the individual at current stage of recovery and the progress them further.

Strength & function and rehabilitation

Targeted strengthening of the muscles needed for everyday tasks – getting up from a chair, climbing stairs, walking to the garden – with realistic goals set from the first session.

Falls prevention programme

A comprehensive, evidence-based falls assessment addressing internal (balance, strength, footwear) and external factors (home hazards, lighting, walking aids). Devising the rehabilitation plan to minimise falls risk and provide rehabilitation to encourage functional recovery following fall.

Assessment of mobility aids

A thorough mobility aid assessment is an important part of elderly physiotherapy as the wrong aid, or incorrectly used aid can increase the risk of falls. Our physiotherapists check, whether the equipment is right for the individual at current stage of recovery and the progress them further.

Equipment assessment and recommendations:

Assessment of aids used for daily transfers which include bed rails, grab rails and toilet/bath equipment to ensure they are appropriate, correctly positioned, and being used safely.

Carer & family training

We keep the elderly individual at the centre of the rehabilitation plan and involve families and carers throughout which includes training in safe moving and handling, how to support exercises at home, and how to help between the sessions.

Ongoing monitoring & modification

We monitor the progress and review it on regular basis according to the goals ensuring quality treatment.
